Class Forms

Please make sure that you turn in your parent/guardian signature form (in Parent Square) acknowledging that you and your parent have reviewed the class syllabus. It will also acknowledge that you and your parent/guardian have gone through the laboratory safety guide and that the student has taken the laboratory safety test, found in the Google Classroom prior to participating in any labs.

The syllabus is located in student's Google Classroom (You will find a PDF version below). The form sent through Parent Square to electronically sign will serve as acknowledgement that you have reviewed it with your student. 

Below you will find the Flinn Safety Contract.  It does NOT need to be printed or signed.  The signature form sent home through Parent Square will serve as acknowledgement.  The student passing the safety test will also serve as acknowledgement from the student.
